BRINGING HEALTH CARE TO YOUR NEIGHBORHOOD
The Community Care Connection is our mobile health vehicle that travels throughout Lake County offering community members a variety of free health screenings, including blood pressure, blood sugar, cholesterol (once a year only when fasting), and body mass index (BMI). The CCC also provides minor sick visits, health education workshops, vaccines for children with Medicaid or uninsured*, uninsured adult vaccines, and more. Screenings are free of charge to adults aged 18 or older, and appointments are not necessary*. Other services are available to minors with parent or legal guardian accompaniment. HELPING YOU DISCOVER BETTER HEALTH
We understand that accessing health care can be difficult for a variety of reasons. That’s why we bring health screening and other services to you. Our team of licensed medical providers don’t just take your blood pressure or check your blood sugar; they also review your test results with you, explain what your results mean and steps you can take to address possible areas of concern.
If you do not have a doctor that you see on a regular basis due to issues such as lack of insurance, the Community Care Connection staff can direct you to appropriate resources.
COMMUNITY CARE CONNECTION SERVICES
The following services are available free of charge:
- Blood pressure measurement
- Blood sugar testing (diabetes screening)
- A1c Testing for those diagnosed with diabetes or prediabetes.
- Body mass index (BMI)
- Cholesterol testing (if fasting for 8 hours. Once a year)
- Minor sick visits***
- Rapid Covid
- Rapid Flu
- Rapid Strep throat
- Pregnancy test
- Check Ears for possible infections or need for irrigation
- Childhood vaccines for children with Medicaid or uninsured*
- Uninsured adult vaccines including Flu, Covid, Shingles, Tetanus/Tdap and more
- Narcan
- Condoms/lubricant
- Sports physicals for children and teens*
- Review of lab results from primary care clinic
- Education regarding medication regimen
- Referrals as needed for additional healthcare services or community resources

Please note that while we encourage individuals to visit the Community Care Connection for these important health screenings, it is not intended as a substitute for having a regular doctor. Should the Community Care Connection medical staff identify an ongoing health concern, they will connect patients with a doctor’s office or clinic that provides more comprehensive care.
WHAT TO EXPECT AT YOUR VISIT
When you first arrive at the mobile health vehicle, please check in with a Community Care Connection staff member. Following a brief registration process, you will be seen by a licensed medical professional. The Community Care Connection also serves as a training site for students enrolled in medical and health professions programs at Rosalind Franklin University of Medicine and Science, and as such, you may have one or more students participating in your screenings. We appreciate your willingness to be a part of their clinical education.
Once your screenings have concluded, a medical professional will review your test results with you, make recommendations based on your results, and provide appropriate referrals to help you establish a regular doctor that can address ongoing health concerns, if needed.
